Information can move from being accurate to inaccurate very quickly.

Time-stamping data helps you figure out what might have happened, but time-stamps don't give you the full context.

While lots of documents have time-based validity (contracts, quotes, etc) that doesn't account for other content in the document that may have changed as drafts of the documents were reviewed.

While version tracking helps, it is also possible that something gets overwritten that should not have been.

Ultimately, inaccurate information is documented and a fair bit of context knowledge is necessary to determine that information captured in the system is inaccurate.
